Telehealth Program

ConfiCare® created a specialized program that utilizes telehealth technology to care for and treat many home health patients. ConfiCare®'s Telehealth Program allows us to clinically manage the patient 7 days per week while giving the patient and their family a sense of security.

Utilizing state of the art equipment from a leading telehealth supplier, our program focuses on monitoring the patient daily by a dedicated triage registered nurse that works closely with the patient, their family and physician to appropriately treat and manage the patient's care while at home. Through this technology and our custom designed protocols, ConfiCare® is able to reduce unnecessary hospitalizations and emergent care visits while improving the patient's quality of life.

Establish 1 to 5 Subjective Questions Related to the Patient's Diagnosis to Monitor Their Status

A Telehealth Triage Registered Nurse to Monitor Data and Communicate with Patient and Physician

Specialized Reports to the Patient's Physician. These Reports Can Also be Used as Objective Proof of time billed under Care Plan Oversight
