Balance Disorder and Fall Prevention Program
ConfiCare®'s Balance Disorder and Fall Prevention Program was designed to assess and treat individuals with balance disorders as well as teaching patients skills to prevent falls and potential injury. ConfiCare® has created a unique program that incorporates effective and efficient clinical pathways, specific clinical tests, specialized treatment guides and equipment for our therapists. We partner with the patient and their physician to help determine the cause of the patient's balance disorder and then create a custom treatment plan that guides the patient through the appropriate pathway and course of treatment.
Our goal of the program is to alleviate/control symptoms, prevent injury and re-hopsitalizations, and ultimately improve the ADL's and quality of life for the patient and their family.
COPD Program
ConfiCare®'s Chronic Obstructive Pulmonary Disease (COPD) Program was created to deliver specialized quality care for our patients who are living with COPD. We have designed the program to educate and guide our patients through the disease process while focusing on solutions to improve outcomes and prevent complications.
Our program is based upon national standards of care and educational materials established by the American Lung Association. By focusing a treatment plan tailored to COPD, we can provide better care and ultimately improve the quality of life for patients living with COPD.
Anticoagulation Therapy Program
ConfiCare® created a specialized program that focuses on the various anticoagulation therapies many home health patients are receiving. ConfiCare®'s Anticoagulation Therapy Program is designed to educate and treat the patient on the various therapies used as well as coordinate this care with the patient's physician.
Our program focuses on assisting the patient understand their particular disease process associated with the anticoagulation therapy while educating them on medication administration and effects, injury prevention and when to contact their physician. By providing this specialized program, we are able to reduce unnecessary hospitalizations and improve the patient's quality of life.
Diabetes Education Program
ConfiCare®'s Diabetes Education Program was designed to educate those living with Diabetes and to improve not only the patient's health, but also their quality of life. After completing a comprehensive diabetes related assessment, we partner with the patient and their physician to develop a custom treatment plan that guides the patient through the entire disease process while educating the patient on items such as insulin injections, blood sugar monitoring and the importance of nutrition and exercise.
We worked closely with physicians and diabetic educators to develop a program whose ultimate goal is to prepare the patient to live with Diabetes while maintaining an acceptable quality of life.
Wound Care Program
Utilizing a multidisciplinary team and partnering with the physician and experts in the wound care industry, ConfiCare® has created a specialized program that focuses on treatment of the wound in the patient's most familiar setting - their home. ConfiCare®'s Wound Care Program has established over 10 wound management protocols based upon industry best practices while utilizing innovative and effective products.
In addition to treatment of the wound, our program focuses on teaching the patient on their wound type, signs of infection, infection control techniques and how to care for their skin. By providing this innovative and flexible program, we are able to heal patient wounds, reduce infections and improve the patient's quality of life.
Urological Program
Utilizing a multidisciplinary team and partnering with the physician and experts in Urology, ConfiCare® has created a specialized program that focuses on services to the urological patient in their most familiar setting - their home. ConfiCare®'s Urological Home Health Program utilizes a flexible protocol that can be customized by the physician to best suit the patient's needs.
In addition to services to the urological patient, our program focuses on teaching the patient on their disease, signs of infection, infection control, and encouraging independence in self care. By providing this innovative and flexible program, we are prepared to reduce infections and improve the patient's quality of life.
Telehealth Program
ConfiCare® created a specialized program that utilizes telehealth technology to care for and treat many home health patients. ConfiCare®'s Telehealth Program allows us to clinically manage the patient 7 days per week while giving the patient and their family a sense of security.
Utilizing state of the art equipment from a leading telehealth supplier, our program focuses on monitoring the patient daily by a dedicated triage registered nurse that works closely with the patient, their family and physician to appropriately treat and manage the patient's care while at home. Through this technology and our custom designed protocols, ConfiCare® is able to reduce unnecessary hospitalizations and emergent care visits while improving the patient's quality of life.
